To obtain a copy of a report on an incident that happened to your vehicle, home, business (or in case of insurance companies, to an insured) please call 386-754-7057 Monday through Friday, between the hours of 8:00 AM and 5:00 PM. We will need the date and address of the incident to pull up the report. Please complete the "Contact Information" section only on the Public Records Request Form and return to Sandra Davis at the Administrative Offices ( or fax 386-754-7064).  Under the Florida Sunshine Records law you may be charged $.15 per sheet for the report. The report may faxed, emailed, or picked up at our office at 509 SW Bascom Norris Dr., Lake City, FL. 32025
